
What exactly is case management?
Social work case management is a process of planning, seeking, advocating for, and monitoring services from different social services or healthcare organizations on behalf of a client.
LASM’s case manager works to advance the well-being of senior individuals in our churches and neighborhood community by improving support systems. This is a collaborative process between the client and the case manager.
Here are some specific examples that our case manager can assist with:
■ Information & referral to a multitude of community resources
■ Healthcare insurance review (Medicare plans, prescription drug plans)
■ Enrollment into Medicare Cost Savings programs, Food stamps, SSI (supplemental security income)
■ Assistance with enrolling/choosing in-home health care services and finding discounts
■ Applying for utility assistance through LIHEAP, MSD, Lifeline, and other community partners
■ Applying for the Missouri Property Tax Credit
■ Linking clients with home repair agencies
■ Applying for financial assistance with hearing aids, dental, vision
■ Completing Advance Care Directives for healthcare and/or Simple Wills (freewill.com)
■ Applying for senior subsidized housing. Overview of housing options when moving to Independent Living, Assisted Living, or Skilled Nursing Homes
